The Basic Information page provides a place for you to enter preliminary owner and accident/violations/watercraft claims history information. modernLINK® accepts up to four operators and one boat or two PWCs on the same quote.

Basic Information

The Basic Information section provides a place for you to enter complete preliminary information that enables you to submit the quote.

To enter Basic Information:

  1. Complete the Basic Information form.

 Basic Information includes:

Label

Description

Company or Corporate Titled

Indicates if boat is owned by a company or corporation. If Yes, enter the Company/Corporate name.

Titled Owner First Name

The applicant's first name as it appears on the unit title. This is the named insured as it appears on the Declarations page. Enter only one name. If joint owners, add the other owner in a full quote as a Joint Owner in the Additional Insured section on the Unit Information page. A spouse should not be listed in this field (add as Operator 2).

Titled Owner Last Name

The applicant's last name as it appears on the unit title. This is the named insured as it appears on the Declarations page.

Home Phone

Applicant's home phone number.

Titled Owner's Birth Date

The Titled Owner's (Operator 1) birth date.

3+ Years of Prior Boat Ownership

Indicates if applicant has previously owned watercraft for three or more years/

Years of Operating Comparable Boats

Indicates if applicant has previously operated similar watercraft for three or more years.

Number of Owners

Indicates if more than one unit owner. A husband and wife are not considered multi-owners.

Is Pro Anglers Endorsement needed (earns > 50% of their income directly through endorsements, payments or winnings from fishing tournaments) ?

Indicates if the Pro Anglers Endorsement is need. This endorsement is required if the applicant earns more than 50% of their income directly through endorsements, payments or winnings from fishing tournaments. Professional Angler coverage is only for customers insuring Bass / Walleye, Pontoon, Runabout and Sport Fishing boats.

Previous/Current Carrier

The applicant's previous or current carrier.

Expiration Date of Current Policy

Enter the expiration date of the applicant's current watercraft policy.

Mooring State

State location where the unit is stored for six or more months per year.

Mooring Zip Code

Zip Code in which the unit is kept for six or more months per year.

Effective Date

The date on which the quote is to be in effect.  modernLINK uses this date to determine current coverages and rate.

When this date is changed, all coverage selections will remain (if those coverages are still available), but the quote is automatically re-rated.

Agency Code

Code assigned to an agency by American Modern.

SubProducer Code

Code assigned to an agency SubProducer by American Modern.

Accidents/ Violations / Claims History

The Accidents / Violations / Claims History section provides a place for you to enter accidents, violations and claims history information necessary for the Quick Quote.

To enter Accidents / Violations Information:

  1. Complete the Accidents / Violations / Claims History form.

Total number of accidents within the last 36 months, including no-fault or at-fault.

Total Number of Major Violations

Total Number of Major Violations within the last 36 months.

Total Number of Minor Violations

Total Number of Minor Violations within the last 36 months.

Total Number of Watercraft Physical Damage Claims

Total number of watercraft physical damage claims made within the last 36 months.

Total of All Watercraft Physical Damage Claims Payments

Total value in dollars of all watercraft physical damage claims payments made within the last 36 months
